HMHP Primary Care Practices Earn National Distinction

Seven HMHP Physician Associates primary care practices have achieved Level III NCQA Recognition. This marks the highest level of distinction for a physician practice. These sites have demonstrated patient centered care focusing on evidence-based medicine and outcome driven results in an environment of teamwork. The practices include:
• Austintown Family Health Center (pilot site that achieved NCQA Recognition in May 2013)
• Boardman Complete Family Health Center
• Church Hill Family Health Center
• Cortland Family Health Center
• St. Elizabeth Family Health Center
• St. Elizabeth Internal Medicine Clinic
• St. Joseph Community Care Center
These practices are the first and only Patient Centered Medical Home (PCMH) sites in the Mahoning Valley.
In a medical home practice, the doctor is the patient’s champion and the practice is the patient’s “home-base” for health and wellness. Working with the staff, doctors lead and coordinate a patient’s overall care. A patient can rely on their doctor to not only treat current health conditions, but also work with them to plan their overall health and wellness, and if needed, coordinate and monitor care from specialists.
For patients, having a “medical home” means better access to their doctor. A PCMH practice offers extended hours and same day appointments to make it easy for patients to get the healthcare they need, when they need it.
